The issue of prevention and treatment of pressure ulcers among the geriatric patients

Abstract: This bachelor thesis deals with the issue of prevention and treatment of pressure ulcers among the geriatric patients. Aims of the thesis: Find out which interventions nurses are practicing to prevent pressure ulcers among the geriatric patients. Explore the nurses' experience in treating pressure ulcers among the geriatric patients. Methodology and research set: To achieve the objectives, research questions were created. What are the interventions used to prevent pressure ulcers among the geriatric patients? What is the experience of nurses in treating pressure ulcers in the selected institutes? The empirical part of this bachelor´s thesis was created using a qualitative research method. The data was collected using semi-structured interviews which were conducted with nurses at selected departments of three hospitals in the South Bohemian Region. The research sample "nurses" consists of 15 nurses in total, each with a minimal experience of 3 years. The interviews were complemented by a long-term direct, participant observation involving 5 nurses from one of the three South Bohemian hospitals in the selected department. The data obtained were processed using the pen and paper interviewing method (PAPI). Open coding was used and the data were then sorted into eight categories and 15 subcategories. Results: The results of the survey showed that the nurses encounter the issue of pressure ulcers among the geriatric patients almost on daily basis. Nurses were able to list the interventions used in the prevention of pressure ulcers among the geriatric patients. Our investigation revealed that some nurses did not take the same action in practice when compared to their statements and described their practices in treating pressure ulcers among geriatric patients only in theory. They are knowledgeable about treating pressure ulcers patients but in practice they do not abide by certain principles. These include the use of protective equipment during bandaging, adherence to indications, communication with the patient, monitoring the patient's general condition, maintaining sterility, supervision and care of the area around pressure ulcers. Recommendations for practice: We recommend the inclusion of more courses about the topic of pressure ulcers in nurse education as it is essential to have sufficient information and regular education to provide quality care. We also recommend that in addition to theoretical knowledge nurses should be given practical training in the prevention and treatment of pressure ulcers.
Pressure ulcer prevention strategies

Aims: The aim of the bachelor thesis was to determine the level of knowledge of general nurses in the field of prevention and treatment of pressure ulcers. Methods: The target group of respondents were general nurses employed at the Intensive Care Unit of the Orthopaedic and Traumatology Clinic of the University Hospital Kralovske Vinohrady in Prague. There were 21 respondents in total. Results: The overall success rate - knowledge of the monitored criteria was 62.33 %. The results of the actual survey are presented in clear bar charts. Comparsion of the approach in the prevention of pressure ulcers in the departments of surgical and non-surgical fields

The aim of this bachelor thesis is to map and subsequently compare the approach in the prevention of pressure ulcers in the departments of surgical and non-surgical fields of health facilities. Today, pressure ulcers are still among the most common chronic wounds, and patients at risk of developing them occur across all wards. The empirical part of the bachelor thesis has been processed by the method of qualitative research. The semi-structured interview technique was chosen for data collection. Interviews were conducted with nurses working in the departments of surgical and non-surgical fields in two South Bohemian hospitals. After processing the obtained data, the results of the research survey were divided into 3 categories, which were further divided into subcategories. The results of the thesis not only provided information on the differences in the approach in the prevention of pressure ulcers among departments of surgical and nonsurgical fields, but a fundamental difference also emerged in the comparison of departments of surgical fields between the monitored hospitals. The differences found concerned the assessment of the risk of pressure ulcers before surgery and the application of preventive measures to prevent pressure ulcers during the perioperative period.The results of the thesis have shown that nurses from the department of nonsurgical fields are much more actively educated in the issue of pressure ulcers, which is also relevant in the departments of surgical fields. Furthermore, it has been found that nurses from the department of surgical specialties, in contrast to nurses from the department of non-surgical specialties, have a deficit of knowledge in positioning patients with hemiparesis or hemiplegia. However, even these patients are commonly encountered by nurses in the operating wards and should therefore be able to provide them with quality care in the prevention of pressure ulcers. The achieved results of qualitative research can serve as an incentive to improve the management of pressure ulcer prevention in health care facilities and thus increase the quality of nursing care provided.
Evidence-based practice in the prevention and treatment of pressure ulcers in intensive care patients

Pressure ulcers are one of the quality indicators of nursing care, their presence reduces the quality of life of the patient, leads to a prolongation of hospitalization, increases costs of care and affects the budget of a health care facility. European Pressure Ulcer Advisory Panel or the European Advisory Board on issues of bedsores (EPUAP) leads and supports all European countries in their efforts to prevent and treat pressure sores. EPUAP working group has developed guidelines for the prevention and treatment of pressure ulcers. One of the important factors in providing the highest quality of nursing care is the ability of nurses to practically use the research results in the nursing process. This thesis deals with the effective use of the latest advances in the prevention of pressure sores and their treatment in the intensive care in Czech Republic. The main objective of this thesis is to explore the latest methods of prevention of pressure sores and care, according to Evidence-based practice principles EPUAP and analyze their practical implementation in selected intensive care wards. Nutritional interventions as a prevention and treatment of pressure ulcers in geriatric clients living in social services facilities Introduction: Pressure ulcers are chronic wounds caused by pressure and friction. This is localized area of cell damage caused by a microcirculation disorder. Prevention is essential especially for immobile clients who are easily creating and developing these chronic wounds. Clinical practice shows that client positioning, use of anti-decubital aids and good nutritional support is necessary. It is important to increase the supply of individual nutrients, especially proteins, and monitor their sufficiency for treatment and prevention. The theoretical part of this diploma thesis was devoted to the etiopathogenesis, classification, prevention and treatment of pressure ulcers especially in terms of nutrition. For my bachelor thesis I chose the area of decubitus.Dekubitus is a serious complicationthat affects all age groups of patients and reduces their quality of life. Occurrence of decubitus is one of the basic indicators of quality of treatment and nursing care and is carefully monitored. Nevertheless, despite the quality and professional care of nursing staff, they arise, as there are factors that can not affect the occurrence of pressure decay. At the stage when there is alreadya decubitus,treatment is coming. Just as today it is no longer possible to treat the decubitus as a separate problem, so its treatment must be conceived in a comprehensiveway. There are a large amount of moneyspent on prevention and treatment of bed sores. The most effective money spent is on prevention that deliberately protects patients from harming their health. In the theoretical part I open the issue of decubitus. Nutritional prevention and the treatment of decubitus of seniors

This bachelor thesis called Nutrition prevention and treatment of decubitus on seniors is about condition of client's diet in Máj České Budějovice Seniors' House and following nutrition intervention on critical clients. Decubitus is a disease, which greatly affects not only sick client or care staff, but also the economic aspect of the medical and social facilities. Because there was no knowledge about, how decubitus originate, the prevention nor treatment in the past, it was considered as very serious illness. Doctors were convinced, that decubitus origins spontaneously and it's a sign of inevitable death. Since the end of the 19th century, more attention has been paid to decubitus, and thus industrial production of phenol-impregnated bandages has been developed to improve the healing of decubitus, together with passive patient activation and surgical wound healing. There are currently many companies on the market that produce a wide range of products and tools to reduce the risk of decubitus or their effective treatment. For example, passive and active anti-decubital mattresses are used for keeping the wet environment in the wound and, last but not least, enteral and parenteral nutritional preparations to help wound healing. The goal of this thesis was to find out whether there are clients with a risk of developing decubitus in the Máj Česke Budejovice Seniors' House and then to process the nutritional intervention of these clients. The theoretical part of the work summarizes the previous knowledge in the field of prevention and treatment of decubitus. As first, the term "decubitus" has been characterized, and the distribution of the individual steps and assessment scales used for the risk assessment or the defects already occurred has been described. There are also mentioned factors influencing the occurrence of decubitus, prevention and nutrition prevention and treatment of decubitus. One chapter of the work was devoted to the NutritionDay project, which was used to collect data for the practical part of the research. A combination of quantitative and qualitative research methods was used to develop the practical part. In the first phase of the research, screening was carried out within the NutritionDay project in the Máj České Budějovice Seniors' Home. The screening revealed how many clients are at risk of malnutrition, how many of them are already malnutritive and how many of them are experiencing decubitus. In the second phase, the work has been done only with clients experiencing decubitus. For each of the clients with decubitus, an adequate daily intake of energy and essential nutrients was calculated. Nutritional support was then set and their diet evaluated in the Nutriservis Profesional program to determine whether it meets the recommended nutritional values. The results of this bachelor thesis can be used by other nutritional therapists for the purposes of teaching or further research.

Nursing care in the healing of pressure ulcers

Bachelor thesis on "Nursing care in healing pressure ulcers" focuses on the care of pressure ulcers compared between public and private health institutions. The aim of this work is to determine the differences in the treatment of pressure ulcers and basic knowledge of nursing staff on the same type of separation. The paper describes the basic anatomical and pathophysiological knowledge of pressure ulcers, and has been omitted or prevention, which is placed particularly in recent years, great emphasis. The largest part is dedicated to the treatment of pressure ulcers, which are referred to as the latest recommendations. In the second part, the theoretical findings using a structured questionnaire, the questions were designed for basic information about prevention and treatment of pressure ulcers.
